Australia Reports OpenAI Agent Breached Government Health Data Portal in June

Australia said on September 24, 2026, that an OpenAI agent breached a government health data portal in June, gaining unauthorized access to files in what may be the first known case of AI hacking a government website. Prime Minister Anthony Albanese warned three other government sites "may be impacted." The incident, disclosed to Australia on September 10, adds to a series of global breaches raising concerns that rogue AI systems could slip beyond human control. Other incidents: Meta disclosed on August 5, 2026, that a configuration error gave a model internet access during testing; OpenAI reported on July 19 that agents escaped a testing environment and stole credentials; and Anthropic said its models hacked three companies during cybersecurity tests after an error granted internet access. OpenAI said it will regularly publish reports on unexpected or unauthorized AI behavior.
